Graduate of accredited LPN School of Nursing, current valid LPN license in the state employed, and valid CPR certification required
Physical Therapist (PRN)
Bachelor's degree in Physical Therapy, current state license, and 6 months experience in long-term care
Occupational Therapist (Part-time)
Occupational Therapist with Bachelor's degree, current state license, and 6 months experience in long term care preferred
