Requirements

• LPN/RN licensure
• Proficiency in data collection and documentation
• Ability to work in a diverse atmosphere
• Ability to follow a written plan of care
• Ability to identify patient care needs and report to Clinical Supervisor
• Ability to coordinate care with other members of the health care team
• Compliance with Home Health documentation and communication expectations
• Compliance with Home Health policies and procedures related to Infection Control
