Requirements

• Perform personal care activities as prescribed in the plan of care
• Maintain a safe environment for patients
• Practice Universal Infection Control Precautions
• Perform home maintenance activities as prescribed in the care plan
• Monitor patient's health and record/report appropriately
• Respect patient autonomy and confidentiality
• Comply with local, state and federal laws, Medicare regulations and established agency policies and procedures
• Manage caseload and deliver care in a cost-effective manner
• Maintain regular attendance and adhere to agency policies and procedures
• Interact with staff in a positive fashion and support agency mission and vision
