Requirements

• Experience in patient care
• Ability to perform personal care activities

• Ability to monitor patient's condition and report changes
• Ability to work in a team and communicate effectively
• Knowledge of local, state and federal laws, Medicare regulations and established agency policies and procedures
• Ability to manage a caseload and deliver care in a cost-effective manner
