Case Manager

Manage client's/patient's/resident’s cases with clinical knowledge and expertise, providing some therapeutic services and coordinating continuum of care activities.

Requirements

• Bachelor's degree in Social Work, Counseling or related human service field
• One or more years' experience working in a behavioral health setting
• Previous experience with the patient population served by the facility
• Previous experience in case management and/or utilization review
