Job Description
This role involves engaging and supporting individuals transitioning into and maintaining independent housing. The case manager will create a Housing Health and Recovery Plan, provide connections to employment, finance, and other supports, and facilitate transition to self-sufficiency. A key component is comprehensive outreach to build relationships within the community and plan for eventual discharge.
Requirements
- Engage and support recipients in transitioning into independent housing.
- Provide comprehensive services addressing life domain areas (employment, finance, relationships, etc.).
- Assist with discharge planning and ensuring successful transition.
