
Access-Supports for Living provides comprehensive vocational, housing, and community-based services designed to enhance quality of life and foster independence for individuals with varying needs.
This role involves engaging and supporting individuals transitioning into and maintaining independent housing. The case manager will create a Housing Health and Recovery Plan, provide connections to employment, finance, and other supports, and facilitate transition to self-sufficiency. A key component is comprehensive outreach to build relationships within the community and plan for eventual discharge.
