Transitional Care Coordinator

We are seeking a Transitional Care Coordinator with lived experience with mental health challenges to work with young adults ages 18-25. The ideal candidate will have a working knowledge of MH, education, social, employment, and community services, and be able to relate to the issues faced by young adults.

Requirements

• Attend SPOA Meetings
• Share lived experiences to support young adults
• Provide direct support, information sharing, and skill building
• Coach and educate young adults to use their voice and make informed choices
• Increase young adult self-empowerment and advocacy skills
• Mediate, facilitate, or bridge between young adults and agencies
• Assist young adults in identifying natural supports, connecting to informal supports, discovering their own strengths, and achieving successful independent living options
