Accord is seeking Case Managers to serve individuals with disabilities in Dakota County, Minnesota, enhancing quality of care and promoting cost-effectiveness. The role involves developing personalized care plans, coordinating care, monitoring progress, and assisting with access to services. Compensation is $45,000-52,000 based on experience and an additional $1,500 sign-on bonus.
