Case Manager - Hennepin County

We are looking for Case Managers to work with individuals with disabilities in Hennepin County. The successful candidate will be accountable for the full individual's care cycle, assessing needs, evaluating treatment options, creating care plans, coordinating care, and monitoring progress.

Requirements

• Bachelor's Degree in Human Services, Social Work, Psychology, Sociology, or a related field
• Current licensure as a Social Worker (if Social Work degree)
• Good written and oral communication skills
• Excellent at working independently and in crisis situations
• Comfortable with high-volume paperwork
• Attention to detail and process oriented
• Ability to pass a DHS background check and have a valid driver's license with a clean driving record
• Reliable transportation
