Community Health Worker

Activate Care is seeking a Community Health Worker to provide care coordination and resource navigation to community members in Michigan. The role involves conducting telephonic outreach, follow-up, and coaching to assist with enrollment in services and benefits, and administering social determinants of health screening and intake forms. The ideal candidate has a passion for working with individuals and families to meet their social and health needs and has experience creating client-centered action plans and connecting them to resources.

Requirements

• High school diploma or equivalent
• Valid driver's license in Michigan
• 2-3 years of relevant work experience providing direct care coordination services
• Experience working with nonprofits, social service providers, faith-based groups, or government agencies addressing social determinants of health
