Community Health Worker (sign-on bonus)

Activate Care is hiring a team of Community Health Workers (CHW) in Nevada to support the screening, assessment, and care navigation for local community members enrolled in the Path Assist program. This hybrid role will help accelerate local change to drive better and more equitable community health.

Requirements

• High school diploma or equivalent
• Valid driver's license in the state of Nevada
• 2-3 years of relevant work experience in direct care coordination services
• Experience working with nonprofits, social service providers, faith-based groups, or government agencies addressing social determinants of health
