Care Manager - Long Island (Bilingual in American Sign Language)

The Care Manager provides person-centered care management services to individuals with intellectual and developmental disabilities, overseeing and coordinating access to services to meet their medical, behavioral health, and social needs.

Requirements

• Bachelor of Arts or Science degree with two years of relevant experience, or a license as a Registered Nurse with two years of relevant experience, or a master’s degree with one year of relevant experience
• Fluent understanding of community cultural traditions, norms, and practices of Members and their families
• Excellent interpersonal, public speaking, and written communication skills
• Ability to work autonomously
• Demonstrate professionalism, respect, and ability to work in a team environment
