We are seeking a certified Nurse Practitioner to conduct in-home wellness risk adjustment assessments for Medicare members and other at-risk populations, assessing overall health and well-being to ensure accurate and comprehensive risk adjustment coding.
Requirements
- Valid unencumbered NP License for the state
- 3 years patient care experience in primary care/adult/geriatric
- Previous in-home risk assessment experience
- Previous 1099, PRN, part-time experience
- Ability to work independently
- Strong ability to work within our EMR system
- Access to reliable transportation
- Bilingual preferred
