Nurse Practitioner - Wake County, North Carolina

Advantmed is seeking a skilled and compassionate Nurse Practitioner to conduct in-home wellness risk adjustments for Medicare members and other at-risk populations in Wake County, North Carolina. This 1099 (PRN) role offers flexibility and the opportunity to directly improve member health and quality of care. The NP will assess health, manage risk, and deliver personalized care to elderly patients in their homes.

Requirements

• Must have a valid unencumbered NP License for the state.
• Preference for weekday or standard working hours.
• Prior in-home risk assessment experience preferred.
• Previous in-home risk assessment experience preferred.
• 3 years patient care experience preferred.
• Access to reliable transportation.
• Strong ability to work independently.
• Bilingual is a plus.
