Job Description
Adventist Health is seeking a Field RN, Care Manager, Home Health to provide in-home care to patients within their homes. This role involves managing patient caseloads, performing comprehensive assessments, implementing care plans, and providing direct patient care. The ideal candidate will be responsible for patient communication, care coordination, and compliance with healthcare regulations.
Requirements
- Registered Nurse (RN) licensure in the state of practice
- Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification
- Valid Driver's License and a reliable automobile
- Current automobile insurance and a reliable automobile
- Basic Life Support (BLS) certification
- Master's degree in Nursing (BSN) (Preferred)
- Acute care facility or home care experience (Preferred)
