Job Description
Coordinates the overall enrollment process by assisting people with the procedures involved in seeking, transitioning, and/or changing agency supports and services. Represents the agency as a community and OPWDD liaison.
Requirements
- Coordinates the overall enrollment process
- Triages requests for information and supports people in the community
- Completes, compiles, tracks, and distributes documentation required for OPWDD eligibility
- Collects data and demographic information
- Functions as a point of contact between agency program administration and individuals seeking services
- Manages, investigates, and presents information based on individual screening packets and OPWDD referral lists
- Facilitates agency program screening, placement, and individual admissions meetings
