Job Description
Jordan Health, an independent FQHC, is seeking an Advanced Practice Nurse (APN) to provide comprehensive primary care and education to patients and families. The role involves identifying community resources, reducing barriers to care, and coordinating services within a multi-problem family setting. The APN will focus on promoting wellness and enhancing patient quality of life.
Requirements
- Must have completed an advance educational program registered by the NYS Education Department or equivalent.
- Licensed to practice as a Certified Nurse Practitioner (CNP) or Board Eligible.
- NYS license.
- Knowledge of assigned area and ability to maintain certification.
