Job Description
Astor’s Prevention Services/Family Connections offers a home-based, evidence-based approach to family needs. The Supervising Case Planner assists case planners in providing direct services like casework counseling, assessments, and crisis intervention, focusing on identifying child welfare concerns and improving outcomes for families. The role involves maintaining records, providing supervision, and supporting staff adoption of the Family Connections model.
Requirements
- Masters degree in a Human Services field.
- Minimum of 3 years in a Preventive Services Setting.
- Bilingual (English/Spanish) preferred.
