RN, Hospice Coordinator / OnCall - Full Time, Nights, Northern / Central NJ

Atlantic Health System is seeking a RN to provide patient visits, respond to medical/emotional crises, and provide skilled nursing care. The role involves providing comfort, compassion, and expertise to patients and families within their homes and communities. Requirements

• Responds to calls from triage and provides patient visits on request of designated triage nurse
• Responds to medical or emotional crisis and/or death as requested by designated triage nurse
• Provides skilled nursing care focusing on pain control and symptom management
• Conducts hospice admissions as needed
• Consults appropriately with attending physician and medical directors
• Assists RNCMs with their patient assignments
