Director Care Navigation

This Director of Care Navigation oversees the daily operations for transition planning for social workers, care navigation, and behavioral health services across the Middle Georgia Region, including Atrium Health Navicent Medical Center, Navicent Baldwin, and Navicent Peach hospitals. The role requires a deep understanding of transitions of care, patient throughput, and cost-effectiveness. The Director supports the care pathway management by ensuring seamless transitions and appropriate resource allocation. They also advise on policies and procedures and identify issues to optimize workflow and improve care outcomes.

Requirements

• Bachelors of Science in Nursing
• Certification / License in Nursing (Georgia or Compact License)
• Experience in acute hospital care with 5+ years in implicit discharge planning/clinical case management
• Knowledge of DRG process and payment methodology
• Proficiency in medical terminology, clinical assessment, and diagnostics
• Understanding of regulatory guidelines for transition planning and Medicare compliance
