Job Description
This Nurse Practitioner position is part of a collaborative agreement with a physician at the AxessPointe Community Health Center in Northeast Ohio. The NP/PA will provide comprehensive primary and behavioral health services to adults, children, and families, utilizing a holistic approach within the community health center network. They will perform clinical, preventative, and educational interventions to promote wellness and improve patient outcomes.
Requirements
- KNOWLEDGE, SKILLS AND ABILITIES
- Formal MSN-NP/PA training and certification in the state of Ohio
- Board certification from ANCC/NCCPA or other valid NP/PA accreditation body with a focus in family medicine
- Licensed in the state of Ohio
- Current DEA Registration
- Compliance with API's Written Standards
