Client Care Coordinator

Boca Recovery Center is seeking a dedicated Client Care Coordinator to support clients through their treatment journey, ensuring successful transitions into aftercare. The role involves managing client care from admission through discharge, with a focus on aftercare planning, engagement, and collaboration with various teams. Responsibilities include developing aftercare plans, facilitating group sessions, and supporting AMA prevention and case management tasks.

Requirements

• Associate’s Degree in Psychology, Social Work, Counseling, or related field
• 1–2 years of experience in behavioral health or substance use treatment
• Familiarity with aftercare planning, FMLA/disability documentation, and case management
• Strong interpersonal, communication, and documentation skills
• Ability to work independently and collaboratively
• Knowledge of community resources and treatment continuum
