Care Coordinator - Street Team

Boston Health Care for the Homeless Program is seeking a Care Coordinator - Street Team to provide complex case management for adults with a history of chronic unsheltered homelessness. The ideal candidate will be flexible, compassionate, and self-motivated, with experience working with vulnerable populations.

Requirements

• Provide outreach-based case management and non-judgmental support to patients with a history of chronic street homelessness
• Engage patients with substance use disorder and mental illness, with a strong commitment to trauma-informed care and harm reduction practices
• Complete health/needs assessments and develop integrated care plans with the patient and primary care team
