RN / Registered Nurse Case Manager - Hospice

Grane Hospice Care is seeking a passionate, dedicated RN Case Manager to join our team! You will have the opportunity to grow your nursing skillset while providing essential one-on-one care in the patients home while supporting their families.

Requirements

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• 1 year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
