Job Description
Adoration Home Health and Hospice is seeking a passionate and dedicated Social Worker to provide expert, patient-centered care to clients in home health settings. Requirements
- Guide and counsel individuals and families in crisis
- Assist clients in obtaining government funds, education, or treatment
- Secure information regarding medical, psychological, and social factors
- Develop and implement viable care plans
- Proficiency in psycho-social assessments
