Job Description
We are seeking a Case Manager - CCR to support patients in navigating health and social service systems, empowering them with the tools, knowledge, and resources they need to advocate for themselves. The role will involve removing barriers to care, promoting viral load suppression, and coordinating patient-centered care.
Requirements
- High school diploma or GED preferred
- Experience in social services, case management, and/or health care will be considered
- Strong computer competency, including knowledge of Electronic Medical Records, Computerized Appointment/Billing Programs and other Microsoft Office Programs
- Strong customer service orientation
