Job Description
Calvary Hospital and @Home in the Bronx, New York, provides palliative care to patients and their families. The Palliative Care Nurse Practitioner serves as a clinical leader, conducting assessments, developing care plans, and coordinating with the interdisciplinary team to improve quality of life and align treatment with patient goals. This role emphasizes patient- and family-centered care and adherence to ethical standards.
Requirements
- Current NYS Registered Nurse license
- Current NYS Nurse Practitioners license
- Master’s or Doctoral degree in Nursing; certification as an Advanced Practice Nurse in Palliative Care or related specialty preferred
- Minimum of two years of clinical experience as a Nurse Practitioner in palliative care, hospice, oncology, geriatrics, or related field preferred
- New York State Driver’s License preferred
- Demonstrated expertise in complex symptom management, advance care planning, and interdisciplinary collaboration
- Ability and attitude relative to the spirit of caring and tradition at Calvary Hospital.
