Requirements

• Conduct initial screenings and assessments to identify clients' behavioral health needs and barriers related to social determinants of health
• Support clients in achieving wellness, autonomy, and improved quality of life by facilitating informed decision-making regarding their diagnosis and treatment options
• Coordinate and monitor all aspects of client care, including care planning, service referrals, and interdisciplinary communication
• Track and monitor client engagement, referrals, service utilization, and outcomes to ensure care plan goals are met
• Identify and address barriers to care by connecting clients to appropriate community-based resources, programs, and services
• Serve as a liaison between clients, families, providers, legal system personnel, and external agencies
