Job Description
Care Dimensions is seeking a compassionate RN Case Manager for a full-time, flexible position. The RN Case Manager will coordinate patient care, develop care plans, and advocate for patients and families, working within a supportive team environment. This is a vital role in expanding access to hospice and palliative care in Massachusetts.
Requirements
- Active Massachusetts RN license in good standing
- Associate’s degree in Nursing or Bachelor’s degree in Nursing preferred
- Strong interpersonal skills
- Valid driver’s license
