Job Description
Care Dimensions is seeking a compassionate RN Case Manager to provide support to patients facing end-of-life illnesses in Massachusetts. The role involves coordinating patient care, developing care plans, and advocating for patients' needs, working under the supervision of a Clinical Manager in a 40-hour/32-hour week schedule. Requirements
- Active Massachusetts RN license in good standing
- Associate’s degree in Nursing required; BSN or equivalent preferred
- Strong interpersonal skills and the ability to thrive in a collaborative team environment
- Flexibility, self-direction, and sound clinical judgment
