Job Description
CareHarmony is seeking a Licensed Practical Nurse (LPN) to work remotely (100%), 80 hours per week. This role involves providing value-based care management to patients, identifying resources, coordinating needs, and educating them on chronic condition management. Requirements
- Active Multi-State/Compact License (LPN) (NLC) (LVN)
- Technical aptitude – Microsoft Office Suite
- Excellent written and verbal communication skills
