The Case Manager Lead provides case management and clinical services to the Crisis Services team, ensuring high-quality crisis intervention and supportive services to individuals and families. They lead the team, provide training, and ensure exceptional care is provided timely and efficiently.
Requirements
- Provides crisis intervention, stabilization, and case management services to clients
- Coordinates treatment with other providers and initiates referrals as appropriate
- Completes progress notes and other required documentation within the required timeframe
- Provides ongoing training, guidance, and field-based coaching to staff
- Fosters teamwork, cooperation, and respectful communication between team members
- Ensures team members adhere to established procedures around clinical documentation
- Promotes a culture of privacy and security of Protected Health Information
