Care Specialist

Provides person-centered assistance to clients in the development of life skills and personal goals, coordinates client care with multidisciplinary team, and connects clients with community resources.

Requirements

• Coordinates client care by working closely with a multidisciplinary team
• Teaches and trains clients in activities of daily living
• Connects client with community support systems
• Provides telephone and on-site crisis intervention/prevention and follow-up to clients and their families
