Job Description
Physical Therapist provides outpatient rehabilitation services in an in-home setting for the older adult population, assessing patient needs and developing individualized physical therapy programs to facilitate rehabilitation.
Requirements
- Valid Physical Therapy license in the state(s) of practice or proof of eligibility
- Master's degree in Physical Therapy, Master's degree in a Physical Therapy-related field plus a Bachelor's degree in Physical Therapy, or Bachelor's degree in Physical Therapy with five (5) years of progressively responsible experience
- Valid driver's license and reliable transportation to patient residences
- Strong organizational and time management skills, effective verbal and written communication skills
