Dietitian (Contractor) - Magnolia Gardens & The Waterford

We are seeking a part-time contracted dietitian to join our team! Providing services at two small Licensed Long Term Care Centres, and reporting to the Resident Care Manager, you will be providing clinical dietitian services for the residents, educating employees and working with the interdisciplinary team to develop personalized nutrition care plans.

Requirements

• Assessing, planning and communicating with the team the nutritional care plan for the residents in the care centre.
• Supporting the Culinary Services Manager / Chef to ensure the provision of high quality, attractive, appropriate meals /snacks in accordance with Licensing requirements
• Direct, Resident Centered Services: Assessment of residents’ nutritional needs and monitoring of their individual nutritional status.
• Work with the interdisciplinary team to develop effective care plans to meet residents’ nutritional goals.
• Provide in-service and ongoing education to staff twice a year.
• Continually evaluate and utilize best practice information/research to improve the quality of nutrition services.
• Compile specified data for reports for performance indicators as requested by the Company.
