Case Manager - Elders & Adults with Disabilities

The Case Manager provides case management services to participants with the Conflict Free Case Management Model, conducting in-home visits, monthly outreach, person-centered plan development, and referrals. The Case Manager collaborates with various departments to ensure participant needs and wants are addressed using a person-centered approach.

Requirements

• Minimum of 2 years' employment experience in a related field
• Minimum 1 year experience in case management role or similar
• Must be able to navigate an electronic medical records system
• Strong organizational, critical thinking and time-management skills
• Basic computer skills required; knowledge of Microsoft Office 365
• Ability to communicate and effectively interact with a diverse population
• Bilingual in Spanish a plus
• Collaboration and teamwork
• Must be able to provide proof of valid driver's license, auto insurance and car registration
• Must authorize Child & Family to complete a motor vehicle records check
• The ideal candidate will be required to meet all background checks and medical clearances required for the position
