Job Description
The Behavioral Health Home (BHH) Nurse Care Manager will work as part of an interdisciplinary team to provide community-based integrated physical and behavioral health services to individuals who may have complex and chronic mental illness, substance abuse, and medical comorbidity.
Requirements
- Current unrestricted RN license
- Valid and unrestricted Driver’s License
- Bachelor of Science in Nursing (BSN) degree (preferred)
- One year of experience (Full-Time) working with adults in a mental health setting (preferred)
