Performs activities which support the Case Management/Utilization Management team function, promoting optimal health care outcomes in accordance with Community First's policies and procedures.
Requirements
- Actively identify alternative practice between physicians, nurses, and other health care individuals.
- Evaluates and documents the patient’s clinical status and post-hospital needs and plans.
- Monitors delivery of care and the patient’s response to care during hospitalization.
- Assesses patient’s physical status to revise plan of care.
- Discusses treatment plan with patient, family and/or significant others.
- Reports/communicates changes in patient and plan of care to staff members involved in patients care.
- Educates the medical staff and other members of the team on proper documentation of services rendered.
- Screens cases for risk management, infection control, or quality of care issues.
- Identifies discharge planning needs, anticipated discharge outcomes.
- Performs admission, concurrent and discharge reviews.
- Communicates and provides updated clinical and medical information and anticipated discharged date.
