Job Description
Provide administrative support to primary care teams in facilitating patient care and promoting positive health outcomes.
Requirements
- Patient Care Coordination in triaging calls and My Chart Messages, routing to appropriate team members and closing communication loop with patients as needed.
- Supporting Quality Measures, ensuring patients are current with all Health Maintenance Goals and follow up visits, ensuring access to care.
- Bridging barriers to care, including arranging for transportation, translation services, coordination with outside agencies and completing paperwork required.
- Documenting and updating computer systems (EPIC, Sorian) to order and coordinate patient services
