Job Description
As a Hospice Aide, you will work under the direction of the Patient Care Manager and registered nurse/case manager to provide personal care and supportive services to patients and families in their homes. This role is part of the multidisciplinary team at Community Hospice & Health Services and its affiliates.
Requirements
- Experience in providing homemaking and personal care services in the home setting
- Valid California Home Health Aide Certificate
- Valid Driver’s License
- Valid California Auto Insurance
- Current CPR certification
- Ability to read and write
- High school degree or equivalent
- Ability to work as a member of an interdisciplinary team
- Excellent verbal communication skills
