Job Description
The RN Case Manager works with patients and families to develop a plan for discharge and arrange post-acute care services. The position assists with gathering data to ensure patient care meets approved admission criteria.
Requirements
- Graduate of an accredited school of registered nursing
- Current CA Registered Nurse (RN) License
- Six (6) months of recent full-time acute care nursing experience in an acute care hospital (RN CM I)
- One (1) year of case management experience (RN CM II)
