Licensed Practical Nurse - LPN

Community Options, Inc. is a national non-profit agency providing residential group home, day habilitation and supported employment services to individuals with disabilities. We are seeking a Licensed Practical Nurse (LPN) in Pottsville, PA.

Requirements

• Provide treatment and health care procedures ordered by a physician and required by the standards of professional practice or law
• Ensure individuals attend medical appointments per their Individual Support Plan (ISP)
• Create and distribute medical/health reports detailing general health, health care goals, and physician recommendations
• Develop and update Individual Support Plans (ISP)/Person Centered Plans (PCP) with an interdisciplinary team
• Train employees on the medical needs of assigned individuals
• Monitor health and medical conditions of individuals and refer for medical services as needed
• Monitor the use of medications through medication administration logs
• Ensure health service supplies are available and replenished as needed
• Ensure program documentation and billable records are completed accurately and timely
• Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families
• Participate with individuals in community, social, and recreational activities
• Participate in circles of support for personal futures planning
• Follow strict OSHA procedures regarding blood borne pathogens
• Employees must cooperate with the licensee and department staff in any inspection, inquiry or investigation
• Additional tasks and responsibilities may be assigned
