Requirements
- Within five (5) days of referral, collect all intake information and enter into agency data collection systems on the day the data is obtained.
- Perform client needs assessments within five (5) days of intake and enter data into CSI data systems on the day the data is obtained.
- Actively participate in Case Planning Team Meetings by presenting new clients, assessments and Individual Service Plan updates on clients as well as discharge planning for clients nearing discharge.
