Community Wellness Ventures (CWV) is seeking two Community Health Workers (CHWs) to join their Community Health team. CHWs will be trusted liaisons between the community, health, behavioral health, and social services, providing outreach, education, advocacy, and support to individuals navigating complex care systems.
