Physical Therapist-PRN

Concept Rehab is seeking a Physical Therapist to provide competent, courteous, and respectful care to patients. The therapist will work within the Scope of Practice for an APT and manage a licensed Physical Therapy Assistant, conducting training and maintaining clinical documentation. Requirements

• Evaluate patients for rehabilitation services within 24 to 48 hours.
• Develop effective treatment plans.
• Treat patients according to the treatment plan of care.
• Communicate with physicians, clinical managers, and other health team members.
• Participate in Plan of Care Meetings.
• Record treatment notes.
• Instruct patient’s families and/or facility staff.
• Recommend equipment for improved delivery of clinical service.
