Job Description
This full-time, eight-hour-a-day position offers a multi-disciplinary role as a Licensed Social Worker/Care Management specialist in a primary in person/telephonic setting. The role involves coaching patients, coordinating care, addressing patient needs, and ensuring proper follow-up after clinical care. It requires collaboration with various healthcare providers and fostering a patient-centered approach to well-being.
