Job Description
The Nurse Practitioner or Physician Assistant will lead a care team focused on providing holistic care to members in managed care plans, handling health risks, and reducing overall healthcare costs. They will work in the home setting, supplementing telehealth visits when necessary. This role emphasizes collaboration and patient education within a risk-based framework.
Requirements
- Licensed in California as a NP or PA
- National Board Certification
- Graduate from an accredited Masters or Doctoral Program
- Ability to use EMR systems and data entry
- Basic PC skills (MS Word/Outlook/PPT/Excel)
- Knowledge of federal and state regulations for Medicare and Medicaid
- Knowledge of community resources access
- Ability to use independent judgment and manage confidential information
