Requirements
- Graduate of an accredited school of medicine with a degree of Doctor of Medicine or Doctor of Osteopathy
- Completion of appropriate Residency training leading to Board Certification in Family Practice and maintenance of active Board Certification status
- Current license to practice medicine in the Commonwealth of Pennsylvania, Bureau of Professional and Occupational Affairs
