MDS COORDINATOR-CERTIFIED

Certified MDS Coordinator for Transitional Care Unit. Schedule Assessment Reference Dates, provide schedules to IDT, and coordinate completion of MDS sections. Ensure timely transmission of assessments and validate reports. Participate in interdisciplinary care plan team meetings and provide education on LTC Perspective Payment System and regulations. Serve as clinical and regulatory resource for staff and track RUG days monthly.

Requirements

• Certified MDS Coordinator-Credentialed certification through AANAC
• Current RN license
• 2-5 years clinical practice experience
• Excellent communication, leadership, problem-solving, and priority-setting skills
