Job Description
Crossroads Health is seeking a School-Based Access Specialist to serve Northeast Ohio communities with behavioral and primary integrated healthcare, early childhood services, and extended housing programs. The role involves screening referrals, data entry, consent management, and diagnostic assessments. This position will be a first point of contact for new and returning patients, engaging in both office and community settings, offering trauma-informed services. The position may be eligible for up to 4 weeks off paid during the summer.
Requirements
- Bachelor's Degree in Social Work or a related field (Master's degree preferred)
- LPC/LSW/MFT license
- Experience performing diagnostic assessments in a mental health setting
- Experience in crisis intervention, triage, and community resource knowledge
- Experience working in or with Educational settings
- Must have an acceptable driving record
- Reliable transportation
- Appropriate insurance (4 pts. or less, $100,000/$300,000 and $50,000 property damage)
- 10-panel drug screening
